Requesting Records in Person

Birth Certificates

Peoria County birth certificates from 1982 to present are available at Peoria City/County Health Department (currently located at the Peoria County Coroner 506 E. Seneca Peoria). Our hours are 8:00am - 4:00pm. To obtain records prior to 1982, contact the Peoria County Clerk’s Office.  

No appointment is necessary. You will need to show a government issued photo ID such as a Driver’s license or State ID.

You have a legal right to a birth certificate if you are:

  • An individual of legal age (18 or an emancipated minor) if requesting your own birth certificate
  • The mother of the child whose birth certificate is being requested
  • The father of the child if you are listed on the birth certificate. A father who is not listed on the birth certificate does not have legal access to a copy of that certificate
  • A legal guardian with documentation to show legal guardianship

Birth Certificate Fees

Fees include $16 for the first copy and $7 for each additional copy. Vital Records accepts payments via cash, money order, cashier’s check, or personal check. Major credit and debit cards accepted and will be charged a service fee to GOVPAYNET.

Death Certificates

Peoria County death certificates from 2008 to present are available at the Peoria City/County Health Department (currently located at the Peoria County Coroner 506 E. Seneca Peoria). Our hours are 8:00am - 4:00pm. To obtain records prior to 2008, contact the Peoria County Clerk’s Office

You have a legal right to a death certificate if you are:

  • The informant listed on the death certificate
  • The next of kin
  • Someone who has a personal or property right interest in the record, and documentation to show this

Death Certificate Fees

Fees include $20 for the first copy and $11 for each additional copy. Vital Records accepts payments via cash, money order, cashier’s check, or personal check. Major credit and debit cards accepted and will be charged a service fee to GOVPAYNET.

Additional Location to Obtain Vital Records

Peoria County birth and death certificates may also be obtained from the Peoria County Clerk’s Office. The Peoria County Clerk’s Office is open Monday through Friday from 8:30 a.m. to 4:30 p.m.
