BIRTH & DEATH RECORDS
The Peoria City/County Health Department issues, records, and maintains birth certificates, death certificates and burial permits. Request records in person or online.
CHILD & FAMILY HEALTH
The Child & Family Health programs promote a healthy family. Programs link families to medical providers, healthcare, and parenting resources. Education promotes healthy food choices and nutrition. Programs include WIC, Childhood Lead Screening, and Breastfeeding to promote healthy families.
CLINICAL SERVICES
The Epidemiology and Clinical Services group provides essential services to protect the community from disease and illness. Services include collecting health status data, providing immunization clinics, and responding to communicable disease such as seasonal influenza, COVID-19, and STI’s. Clinical Services also host Heartland Health Services at our location for exams.
COMMUNITY HEALTH POLICY AND PLANNING
The Office of Community Health Policy and Planning participates with community partners in advancing population health improvement and evaluating the impact of health issues in Peoria County. Programs address the Community Health Needs Assessment and Plan, as well as promote an equity framework for many community grants.
DENTAL HEALTH
The Dental Health Clinic works to prevent dental disease, promote oral health, and protect the community with a competent workforce.
